That is a tough way to feel. From the information you have shared there may be a reason that you have not thought of. It may be that she doesn't feel that she is qualified to help you any further. You well may have made improvements since last year but she may not be able to help you continue with the improvement.
It is rather like piano lessons, some teachers are really good at helping piano students get started in the right direction but are unable to help students advance to the higher skill levels. She may recognize her own limitations and wants you to have the best guidance that is available to you.
Rather than put her on the spot, just thank her for the help she has given you and ask her for a referral to someone in the community.
